Non-Discrimination Policy
Affinity Support Coordination, LLC complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex.
Affinity provides:
Free aids and services to people with disabilities to communicate effectively (e.g., qualified sign language interpreters, written information in other formats when necessary).
Free language services to people whose primary language is not English (e.g., qualified interpreters, information written in other languages).

Freedom of Choice
You have the right to choose your Support Coordination Agency and direct service providers.
You may change your Support Coordination Agency at any time.
Participation in Planning
You have the right to take part in the development of your Plan of Care.
You may include family members, guardians, or other representatives of your choice in the planning process.
Privacy & Confidentiality
Your personal information and records are confidential and will only be shared as required or permitted by law.
You have the right to review your records upon request.
Respect & Non-Discrimination
You have the right to be treated fairly, respectfully, and without discrimination on the basis of race, color, religion, national origin, age, disability, sex, gender identity, sexual orientation, or any other protected status.
Health & Safety
You have the right to receive services in a safe environment, free from abuse, neglect, or exploitation.
You have the right to report concerns without fear of retaliation.
Complaints & Appeals
You have the right to file a complaint with Affinity Support Coordination, LLC at any time.
You also have the right to file a complaint directly with the Louisiana Department of Health (LDH).
